As an independent private practice our office participates with many insurances including, but not limited to many Highmark, UPMC and Medicare insurance plans. As there are many different plans available in the market today it is difficult for us to determine your exact plan of coverage, and therefore you may be responsible for yearly deductibles or co-insurance payments.
To ensure coverage for your appointment we encourage you to contact your insurance company regarding their policies. We would be more than happy to provide our tax identification number for you to provide to your insurance company so that you can check coverage. Please be aware that any unpaid balance remains your responsibility.
We ask that you bring your insurance cards and a photo I.D. to our office for every appointment and keep us updated regarding any coverage changes.
